Units attacking from a fortified position (cities, citadels and forts) will not follow-up into the hex if they kill the defender, but they will remain within their walls.
Also fixes the bug where a melee unit may not attack from a city if another unit is present in the city (including ships) but a ranged unit may. Solves the problem where you have to move a ship out of the city before a ground unit can attack, and also the issue where a newly built ranged unit may fire (and use up its moves) even though the stacking limits are exceeded.
Also fixes the bug where a unit that can move after attacking may not capture a civilian unit, eg the worker in the barbarian camp where we just destroyed the defender
